Department Profile: The Children's Health Center and Lenox Baker Children's Hospital at Duke University Hospital is comprised of pediatric outpatient clinics. Our team is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time.
Department Profile: The Complex Care Clinic at Duke Children's is active in delivering exemplary care for each patient to ensure an excellent patient experience.
This position will provide triage support and care coordination for this high acuity patient population in collaboration with dynamic multidisciplinary teams. This position will work closely with the faculty directors of Pediatric Complex Care Team. We are looking for someone with the ability to establish a strong rapport with patients and families by phone, coordinate services, and provide education and nursing support to patients and families in and out of the clinic setting.
The ideal candidate for this position will possess the following:
A solid background in Pediatric Nursing; specifically neurology
Experience with telephone triage or managed care coordination
Flexible attitude with the ability to function both independently and as a team member, assisting patients/ families through home care coordination and in the clinic setting
Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others
Triage calls resolving problems within scope; gather relevant information and coordinate follow-up with providers to develop plan and convey plan back to patients. Attend outpatient clinic as needed weekly. Document communication in Maestrocare and manage inbasket messages regarding care.
Field pages from outside clinics and patient/families on rotational basis (daytime hours only).
Prior authorizations and drafting letters of medical necessity
Coordinate renewal of outpatient prescriptions
Round on inpatients with attending physicians as needed and facilitate coordination of care to outpatient.
Help coordinate diagnostic testing and outpatient appointments here at Duke, or at an outside facilities
Work Hours & Shifts: Full Time: Hours may vary, but will typically fall within a standard Monday-Friday, 8am-5pm schedule.
