Overview

St. Luke's Health System is seeking a Nurse Navigator to join our OBGYN Support Services / Women's Service Line team.

The RN coordinates for patients with fetal abnormality to ensure the patient is being seen by the right provider. The Nurse Navigator works closely with Obstetric providers, Maternal Fetal Medicine, Neonatology, Pediatric Surgery, Pediatric Urology, Pediatric Cardiology, and Genetics Counseling to care for patients with a diagnosis of fetal anomaly.

Monday - Friday, 8am - 4:30pm.

Responsibilities

• Serves as a single point of contact for patients undergoing treatment,.
• Serves as a liaison between the patient and their family and the patient's network of care providers.
• Educates patient on diagnosis, treatment options, and prognosis.
• Assists the patient in scheduling appointments, completing paperwork, or securing referrals.
• Facilitates short term and/or long term interventions.
• Possesses comprehensive knowledge of subject matter.
• Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
• Other duties and responsibilities as assigned.

Qualifications

• Education: Bachelors of Nursing Degree
• Experience: 2 years's of RN experience
• Licenses/Certifications: Current RN License in the State of Practice

Preferred Qualifications:

• Prior experience and a passion for caring for and supporting patients with high-risk pregnancy.
• Good organization, communication, and care coordination skills.
