SUMMARY: The RN Navigator is a specialty nurse who functions in the role of expert clinician, educator, consultant and coordinator of care. In collaboration with the Providers, Nurses and the healthcare team, the RN Navigator coordinates, monitors and evaluates the clinical care and services for patients and families providing clinical support to improve access to care, patient care, and outcomes. The RN Navigator serves as a resource and primary contact to assist the patient and family in navigating healthcare and the course of treatment. With in-depth knowledge and skills, advanced judgment and clinical experience in the area of specialty, the RN Navigator assists the patient, family, staff and physician in providing solutions for complex patient care issues and in navigating the healthcare systems. The RN Navigator coordinates activities across; the continuum of care, interdepartmentally, and multiple healthcare facilities and subspecialty providers. This position practices within the scope of the California Nursing Practices Act and in accordance with healthcare organizations policies and under the direction of the Chief Nursing Officer.
The Genetics Clinic services include comprehensive physical exams, analysis of family history, genetic testing and genetic counseling. We provide both in-person and telemedicine consultations and review inheritance, prognosis and treatment, when available, for people diagnosed with genetic conditions. Our team includes board-certified and board-eligible physician geneticists, licensed and certified genetic counselors, nurse coordinators, as well as a metabolic dietitian and social worker.
The team is certified as a California Children's Services (CCS) Metabolic Treatment Center for the CA Newborn Screening Program. The team works with reproductive medicine as part of the prenatal diagnostic center care team at the Maternal Fetal Center.
Graduation from an accredited Nursing program required, Bachelor's Degree in Nursing (BSN) highly desirable
Active California RN License
Minimum 3 years' experience in clinical nursing specialty, pediatrics and/or genetics experience a plus
Extensive, advanced knowledge of nursing principles, clinical practice, and healthcare systems is required
POSITION DETAILS: Part Time, 64 hours per pay period, exempt level position. Clinic closed on weekends and holidays.
