DescriptionUNIT DESCRIPTIONThe Transitional Care Center (TCC) cares for a complex, high-acuity patient population. Patients on this unit require high ventilatory/respiratory support (trach/vents; CPAP/BiPAP) while also having other complex medical needs, resulting in interdisciplinary care team appraoch to their care. There is a highemphasis on patient/family education on this unit, as we are transitioning patients from ICU level care to home and families need to learn how to care for their child's complex care needs in the home setting. The primary population on the unit are NICU gradutes, with a smaller population on the unit coming from the PICU or CICU.REPRESENTATIVE RESPONSIBILITIESCompliance - Patient Services Staff
