The VA Northeast Ohio Healthcare System is recruiting for one Licensed Practical Nurse (Women's Health). The Licensed Practical Nurse (Women's Health) will function in Nursing Ambulatory Care Service. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must be a graduate of school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing. Must possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-7: Completion of at least 1 year of qualifying experience at the GS-6 level or equivalent fully meeting all performance requirements for the GS-6 LPN/LVN. Applicants must also meet the following: Technical proficiency in initiating, performing and completing assigned duties in providing care to variable patient populations. Skill to carry out more specialized patient care duties, including assignments in selected, more complex inpatient and/or outpatient care settings. Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery. Ability to proactively assist the RN or MD/DO in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments according to established policies/procedures and appropriately recording administration and patient's response to medications/treatments received. Knowledge and ability to recognize urgent or emergent patient care situations. Ability to proactively assist the RN or MD/DO in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments according to established policies/procedures and appropriately recording administration and patient's response to medications/treatments received. Possesses comprehensive practical nursing skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care. Supports the RN or MD/DO in monitoring the practice of unlicensed nursing or non-medical staff in patient care delivery. Work of considerable difficulty and responsibility is performed under general supervision. Actively seeks out educational opportunities to enhance nursing knowledge and skills, and providing leadership in assisting other staff improve and advance their nursing practice. Promptly reports any deviations from normal practice patterns to the RN or MD/DO. Possesses the ability to assist nursing (RNs) and medical staff (MD/DO/PAs/ NPs) to appropriately prioritize overall Veteran care needs and make adjustment to the plan to deliver care. Demonstrates the ability to make astute observations of subtle, less obvious changes in condition and can accurately document care delivered in acute/urgent/stat conditions and take the necessary steps to ensure Provides indirect nursing care to patients with complex needs as related to consult management, screening of telephone calls, as well as assuming responsibility and accountability for nursing action. Demonstrates the ability to establish priorities, manage multiple tasks, and provide a professional response to other facilities and callers. Tracks facility wide breast and cervical cancer screenings in collaboration with Primary Care Providers (PCPs), Patient Aligned Care Teams (PACTs), and WH liaisons to include: notification of patients who are due for screening, tracking of completion of screening, results reporting/documenting, alerting to provider/PACT team, and follow-up care utilizing Computerized Patient Record System (CPRS) as well as local and national mammography registries.
