Work Hours/Shift: Part Time (Monday - Friday 7am - 12pm)
You Will Be Responsible For:
Advises Rehabilitation Admission Clinical Liaison of referrals to ensure response time is within twenty-four (24) hours of referral.
Establishes and maintains methods of communications with referring and supporting health care individuals and organizations to ensure clear and concise exchange of necessary information in a timely manner.
Verifies patient census daily to determine bed availability. Maintains data base on all referral’s sources, care managers, competitors and physicians.
Performs eligibility and benefits verification applicable to patient’s admission. Uses utmost caution that authorizations and pre-certifications are correct and as accurate as possible to avoid rejections and/or denials. Maintains a current and thorough knowledge of utilizing online eligibility and pre-certification tools made available.
Enters and updates all referrals information into the tracking tool.
Assists with the admission of the patient to ensure a smooth transition through the continuum of care. Informs the care manager that discharge orders are written and transportation is arranged, and confirms patient is still medically stable. Advises the receiving unit of the approximate time of admission and informs capacity management.
What You Will Need:
Computer skills to include Outlook, Microsoft Word, Excel and Power Point
Ability to communicate professionally with an acceptable use of English grammar
High School Diploma or equivalent GED. (Preferred)
The Admission Clinical Liaison Assistant collects patient referral information for admittance to the Inpatient Rehabilitation Unit. Assists with admissions, keeps accurate records of all referrals, and completes monthly reports.
