Position Summary:

Transforms care through assessment and advocacy, decreasing barrier (i.e., insurance, language, prep compliance) to access, coordinating and utilizing appropriate resources and providing a positive financial impact. An effective RN Nurse Navigator possesses skills in communication, organization, problem solving, critical thinking, collaboration, advocacy, time management, and multitasking. Must possess knowledge and experience in the department specialty and have a broad understanding of the health care system. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.
Accountability:
PCMH/PCSP - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable CUSTOMER SERVICE - Ensure quality customer service through training, monitoring and coaching; resolve customer service issues and problems NAVIGATION/ PATIENT ADVOCACY - Work to eliminate barriers to access to culturally competent medical services for all patients. Assist patients to access community resources, and financial assistance programs and services that are needed COLLABORATION - Collaborate with medical providers, multidisciplinary team members and community resource providers to assist patient to meet above described treatment goals PROBLEM SOLVE â Recognize problems, identify the cause and take corrective action; Inform leadership of activities, problems and needs within the department POLICIES - Work within Hospitals, Nursing division and departmental policies STATISTICS - Maintain and report applicable statistics regarding programs and client services DOCUMENTATION- Document all client encounters per the electronic medical record as instructed COMMITTEES and MEETINGS â Attend meetings; participate in committee functions; participate in hospital-wide activities and committees within the scope of position or as directed DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
Education Requirements:
BSN or MSN
Experience Requirements:
1 year directly related experience
Licensure/Certification Requirements:
CPR Certification for Healthcare/BLS Providers or for Professional Rescuers or must obtain within 30 calendar days of date of position Licensed Registered Nurse (RN) in State of New Mexico or as allowed by reciprocal agreement by State of New Mexico
Physical Demands Requirements:
Sedentary Work: Exerting up to 10 pounds of force occasionally (Occasionally: activity or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
Testing Requirements:
Tuberculin Skin Test required annually
Working Conditions Requirements:
May be required to travel to various work sites May be required or is required to perform on-call duties May be required to perform subordinate tasks in high census/vol No or min hazard, physical risk, office environment Sig Haz: Physical risk, injuries due to combative patient behavior
